Kernel.Vmm: Remove hack from #2726 (#3864)

* Remove SceKernelInternalMemory mapping

Contrary to my initial beliefs, this is very much a hack.

* Unreachable for unpatched code

This will always infinitely loop, making logs extremely large.

* Update linker.cpp
